YOUTH MEMBERSHIP FORM
Youth membership is available to any youth 19 years of age or
under who aren't part of a family membership.  The age of the
youth exhibitor is determined by the age on January 1 and will
show at that age for that calendar year.

Annual Youth Membership dues are
$5.00, payable to GHMHC.

When joining for 2010, please do the following:

  1. Fill in all lines below. Please make sure all
    information is correct - this is what will be
    included in the Membership listings.
  2. Print the page (one for your records, on additional
    if paying by check).
  3. Click on the "Submit Form" button to email your
    info to the Web Coordinator. When you click on
    "Submit Form," you'll see a review of the
    information you entered. Simply click on the
    "Back" button to return to this page.
  4. Pay Dues

    the completed form & your membership
    payment to GHMHC at the address shown.
